NutritionRx ("we," "us") respects your privacy. This policy explains what information we collect on this website and how we use it. It is not a substitute for the HIPAA-compliant patient privacy notice you will receive if you become a patient of the practice.
Information we collect on this site
When you submit the qualification form at /qualify, we collect the contact and fit-screening information you provide: name, email, phone, age range, BMI range, conditions you select, prior weight-loss attempts, timeline, and the answers to the questions presented in the form. We do not request, and you should not submit, detailed protected health information through this website.
The site uses privacy-respecting analytics to understand aggregate traffic. No personally identifying information is sold or shared with advertisers.
How we use it
Qualification submissions are reviewed personally by Alexis and her clinical team to determine fit. If your qualification proceeds, you will be onboarded onto a HIPAA-compliant clinical platform separate from this website, and a full Notice of Privacy Practices will apply.

This website uses cookies and similar tracking technologies, including analytics cookies and advertising pixels (such as the Meta pixel), to understand how visitors use the site and to measure the performance of our advertising. If you provide your mobile number and check the consent box on our qualification form, you agree to receive text messages from NutritionRx related to your inquiry and appointments, and, where you separately opt in, occasional promotional updates. Message frequency varies. Message and data rates may apply. You can opt out at any time by replying STOP, or reply HELP for assistance.
Mobile information and SMS opt-in consent are never sold, rented, or shared with third parties or affiliates for their own marketing purposes. Marketing consent is collected separately from any transactional consent and is not a condition of becoming a patient or receiving any service.
Data security
We take reasonable administrative, technical, and physical measures to protect the information you submit. Information sent through this website is transmitted over encrypted 256-bit TLS connections and stored on access-controlled systems, accessible only to authorized members of the NutritionRx team. No method of transmission or storage is completely secure, but we work continually to safeguard your information. Detailed health information you provide after becoming a patient is handled separately on a HIPAA-compliant clinical platform.
How long we keep it
Qualification submissions are retained for up to 24 months. You may request deletion at any time by emailing the practice.
Your rights
You may request access, correction, or deletion of any personal information you have submitted to this site. To make a request, contact the practice using the information in the footer.
